Chicago Lab Services will provide physician offices and nursing homes with specimen collection supplies and containers at no cost. We will provide blood collection tubes and urine tubes, needles, and needle holders. At Chicago Lab Services, we believe that the quality of specimens is strongly correlated with quality results. Therefore, we require careful specimen collection and tagging. Each specimen should have the patient’s name, date and time of collection, and specimen type. We require that specimens be accompanied by a requisition. Each requisition should contain the patient’s name, age, sex, date specimen was drawn, test requested, and patient billing information. For effective transport of specimens, we suggest that labeled blood/serum/urine tubes be wrapped with absorbent packing (e.g., dry-lock pads) or inserted into a rigid plastic container. Please wrap the specimen vials with the requisition form. The plastic container should be placed into the corrugated cardboard box provided for shipping. Please do not submit non-frozen samples with frozen samples in the same requisition.
